The C-arm (mobile C-shaped X-ray machine) is widely used in surgeries such as orthopedics and neurosurgery, providing real-time imaging guidance. However, due to its complex structure and inability to undergo complete sterilization, improper intraoperative handling may lead to contamination of the sterile field or increased risk of radiation exposure. Therefore, disposable sterile protective covers are commonly used to shield critical parts of the C-arm (such as the X-ray emitter and receiver) to prevent direct contact with the sterile area.
All personnel must receive training on the proper use of C-arm protective covers and pass an assessment in aseptic technique. Below is a guide on the correct usage of C-arm protective covers during surgery:
1. Preoperative Preparation
Equipment Check:
- Ensure the C-arm functions properly, with no faults in the power supply or image display system.
- Verify that the protective cover packaging is intact, properly sterilized with ethylene oxide, undamaged, and within its expiration date.
2. Installation Steps
Opening the Package:
- In the sterile field, use sterile forceps to open the protective cover packaging to avoid direct hand contact with the inner sterile surface.
Covering the Device:
- Begin wrapping the cover from the X-ray emitter end of the C-arm, ensuring complete coverage of the C-arm and key operational components.
- Use elastic bands or clamps to secure the cover and prevent displacement during surgery.

3. Intraoperative Handling Guidelines
Aseptic Maintenance:
- Once the cover is installed, avoid moving the C-arm across the sterile field. If crossing is necessary, an assistant should hold the edges of the cover, keeping the sterile surface facing upward.
- During fluoroscopy, expose only the necessary surgical field while keeping the rest covered.
Position Adjustment Coordination:
- When adjusting the patient’s position, the surgical team should coordinate with the movement of the C-arm to prevent tearing or dislodging the cover.
Emergency Measures:
- If the cover is damaged or contaminated, replace it immediately with a new sterile cover and re-disinfect any contact areas.

4. Postoperative Handling (Cleaning and Maintenance)
- Dispose of the single-use cover as per medical waste regulations. Do not reuse.
- Clean the surface of the C-arm with non-corrosive cleaning agents, ensuring no liquid seeps into internal circuits.

C-Arm Design and Infection Prevention
In addition to disposable sterile covers, product design also plays a key role in minimizing the risk of intraoperative infection. For example, the Perlove Medical PLX C7600 Series mobile flat-panel 3D C-arm features a cable-free exterior design. By integrating cables within the machine’s structure, it avoids the issues associated with external cables being pulled or displaced along the track. This design helps maintain a sterile environment and makes it easier to apply the sterile cover.
